Perform comprehensive head-to-toe physical assessments and manage medication reconciliation for hospice patients. Create and update Plans of Care while ensuring strict adherence to CMS Conditions of Participation and state regulations.
Must hold an active, unencumbered RN license in the state of California with deep expertise in hospice regulations and LCD guidelines. Requires strong clinical assessment skills and proficiency in EMR systems for timely documentation.
