The RN Case Manager provides hands-on hospice nursing care and coordinates services for patients and families. Responsibilities include assessing client eligibility, managing personalized care plans, and participating in on-call duties.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems and a valid driver's license with dependable transportation are required.

Schedule: Monday - Friday, 8am - 4:30pm
Territory: Conway
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.
