Responsibilities

The RN Case Manager directs and coordinates quality care for hospice patients while ensuring regulatory compliance and safety. They are also responsible for providing essential education and training to primary caregivers regarding the patient's plan of care.

Requirements

Candidates must be a Registered Nurse with a valid Arizona license and CPR certification, with a BSN or CHPN preferred. A valid driver's license and the ability to perform heavy physical work, including lifting up to 100 pounds, are required.

Full job description

General

The RN Case Manager holds a crucial role in directing and confirming prompt quality care for hospice patients. The RN Case Manager directs, and coordinates activities involved in hospice patient care by performing the following duties personally or through subordinate staff. Ensures regulatory oversight compliance for response, care, documentation, ongoing education/CEU’s, confidentiality, security, and safety. Provides sensitive and thorough education/training to patient’s primary caregiver(s) for proper participation of their care responsibilities identified in POC. Ensures the overall direction of company operations with the highest degree of company expectations, industry standards and legal compliance.

 

Minimum Qualifications (Knowledge, Skills, EDUCATION, and Abilities)

  • Graduation from an Accredited RN program; BSN or Advanced Education or certification preferred
  • CHPN preferred.
  • Maintain current Arizona RN License and any applicable CEU’s
  • Maintain current CPR Certification
  • Strong interpersonal and communication skills (written and oral), including the ability to lead discussions in diverse groups of varying size.
  • Well organized, detail oriented and able to work independently with minimal supervision.
  • Advanced level competency in electronic medical record software.
  • Must possess and maintain; a valid AZ driver’s license, clean driving record, and maintain company insurance qualification of minimum coverage as follows: $100,ooo Per Person Bodily Injury, $300,000 Per Incident and $50,000 Property Damage Personal Insurance Policy.

DOL Strength Rating Heavy Work - The employee must occasionally lift and/or move up to 100 pounds, frequently lift and/or move up to 50 pounds and/or up to constantly lift and/or move up to 10 pounds.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Job Exposure Risk Category 1 – All job-related tasks involve an inherent potential for body fluid exposure. Personal Protective Equipment and safe work practices MUST be utilized as applicable.
