Requires a high school diploma, current Kentucky State Registered Nurse Aide certification, and a valid driver's license. Candidates must have CPR certification and at least one year of experience with frail and elderly populations or be willing to undergo training.

Position Summary:
The PACE Home Care Certified Nursing Assistant (CNA) provides direct personal care and supportive services to participants in their homes, supporting independence, safety, and quality of life. This role focuses on delivering one-one-one care in a home setting, including assistance with activities of daily living, light housekeeping, meal preparation, and observation of participant condition.
Working under the supervision of a Registered Nurse (RN), the Home Care CNA plays a key role in identifying and reporting changes in participants’ physical, mental, or environmental status to ensure timely intervention and continuity of care. The position requires the ability to work independently in the field while maintaining strong communication with the Interdisciplinary Team (IDT).
