Provide supportive services to meet the social, emotional, and financial needs of patients and families facing terminal illness. Conduct psychosocial assessments and provide individual and family counseling regarding end-of-life care and grief.
Requires a Master's degree in Social Work from a CSWE-accredited school and at least one year of experience in an acute health care setting. A valid California Driver's License and automobile insurance are required.

We are seeking a Social Worker to become a member of the hospice transdisciplinary team, will be responsible for providing supportive services to help meet patient/family social, emotional, and financial needs related to the impact of a terminal illness, loss, and grief.
