The Habilitation Specialist delivers individualized, person-centered services to help members acquire skills and build independence. They are responsible for implementing goals, facilitating community activities, and maintaining accurate session documentation.
Candidates must have a high school diploma or GED, a valid Arizona driver's license, and reliable transportation. At least one year of experience working with individuals with intellectual or developmental disabilities is required.
Job description:
The HAB Access Specialist is a field-based member of the Home Assist Health Habilitation team. This role exists to deliver individualized, person-centered habilitation services to members under Arizona DDD/AHCCCS programs — helping them acquire skills, build independence, and achieve the goals outlined in their Person-Centered Support Plan (PCSP). This is a direct service role. The HAB Access Specialist's primary job is to show up for members, implement their goals with fidelity, and document sessions accurately. This role requires a driver who is flexible, reliable, and genuinely committed to person-centered care
