Deliver exceptional and individualized care as determined by each resident’s care plan
Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity
Report changes in resident health status to the clinical team
Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences
Comply with all rules and regulations regarding confidentiality and privacy of resident information
Perform any other duties as assigned
High School graduate or GED equivalent required
Current certification from an accredited Certified Nursing Assistant (CNA) program
Exceptional communication and interpersonal skills
Must demonstrate the ability to serve with love, compassion, and dignity

The community, which is designed to serve adults 65 and older of all incomes who need some help to maintain their independence, is located on on the southwest corner of McLean Blvd. and Bowes Road, between Hwy. 31 and Randall Road in South Elgin. In addition to the assisted lifestyle community, the Heritage Woods campus features two memory care communities: White Oaks at Heritage Woods of South Elgin and White Oaks at Spring Street.
