Position Description

The Nurse Practitioner - Substance Use Health is a key member of the Mental Health and Addictions Treatment (MHAT) Hub, providing client-centered, trauma-informed, and culturally safe primary health care to individuals experiencing complex challenges related to substance use, homelessness, and mental health.

Working within an interdisciplinary team, the Nurse Practitioner provides comprehensive assessments, treatment, and health promotion across the MHAT services, including the MHAT Hub, 24/7 residential treatment, mobile, drop-in, outreach, street health, and collocated services. This role ensures equitable and accessible care by addressing the social determinants of health and implementing innovative, community-based approaches to primary care delivery.

Key Responsibilities:

Clinical Care & Client Support

  • Working withing the full scope of a Nurse Practitioner, conduct assessments, diagnoses, treatment, health education, and counseling in alignment with the College of Nurses of Ontario (CNO) standards for Nurse Practitioners (Extended Class).
  • Provide ongoing monitoring, chronic disease management, and follow-up care for clients. Respond to episodic illnesses, complex cases, and crisis situations, ensuring timely interventions.
  • Maintain accurate and comprehensive documentation in electronic medical records (EMRs), participate in chart reviews, and engage in case conferencing.

Interdisciplinary Collaboration & Coordination

  • Work closely with interdisciplinary team members and external service providers to ensure coordinated, holistic care.
  • Develop, implement, and evaluate treatment, education, counseling, and health promotion programs for individuals, families, and the broader community.
  • Collaborate on screening programs, workshops, and health initiatives to address community needs.
  • Support outreach efforts to enhance access to primary health care, including mobile services and off-site visits.
  • Participate in community engagement and partnership tables as directed.

Continuous Improvement

  • Provide supervision and mentorship to students, graduates, and learners, aligning with HUCCHC’s commitment to education and professional development.
  • Identify gaps in protocols and procedures, contributing to quality improvement initiatives.
  • Participate in committees, team, staff meetings, and program evaluations to enhance service delivery.
  • Maintain professional competency through continuing education, research, and peer collaboration.
  • Support systemic improvements to address barriers to care, particularly for marginalized and underserved populations

Other

  • Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses, etc.).
  • Work within the anti-racism, anti-oppression framework at Hamilton Urban Core.
  • Participate in Centre-wide activities such as the annual general meeting, client centered activities, annual events and all other special events and activities related to Hamilton Urban Core
  • Other duties as assigned.

Performance Targets

  • Serve approximately 10-12 clients per day based on a 7.5-hour workday (1.0 FTE).
  • Maintain 30-minute appointment durations, with flexibility for complex cases, mobile visits, and cultural interpretation needs.

Qualifications

Education & Registration

  • BScN with Nurse Practitioner certification and registration with the Ontario College of Nurses (Extended Class). Experience & Skills
  • Broad-based nursing experience with strong clinical, assessment, and problem-solving skills.
  • Demonstrated ability to work effectively in primary health care, particularly in community health settings.
  • Experience providing trauma-informed, harm reduction, and equity-based care.
  • Sensitivity to issues affecting clients accessing HUCCHC, including those related to mental health, substance use, and homelessness.
  • Experience working with immigrants, refugees, seniors, youth, and low-income populations.
  • Knowledge of government-funded services and supports for individuals with complex needs.
  • Familiarity with the Community Health Centre sector is an asset.
  • Strong interpersonal, communication, and collaboration skills.

Schedule:

8 hour shifts/ Monday - Friday 9:00 am-5:00 pm, 1 day per week 12:00 pm-8:00 pm/ Occasional Saturday 9:00 am-2:00 pm
