The LVN provides physical, psychosocial, and environmental care to geriatric patients in their homes and partnering communities. Responsibilities include implementing physician-ordered care plans, documenting clinical notes, and providing patient and caregiver education.
Candidates must be graduates of an accredited vocational/practical nursing school with an active state license and current CPR certification. One year of clinical care experience is required, with home health and Homecare Homebase experience preferred.

Requirements
Education: Graduate of an accredited school of vocational/practical nursing.
Licensure: Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure
Experience: One year of experience as a licensed vocational/practical nurse in a clinical care setting required.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification.
Transportation: Reliable transportation and valid auto liability insurance.
