The Case Manager is responsible for developing, implementing, and evaluating patient care plans while coordinating the continuum of care. They also manage clinical data, facilitate communication between the treatment team and insurance providers, and may conduct individual or family educational sessions.
Candidates must hold a Bachelor’s degree in Social Work, Counseling, or a related Human Service field and possess AOD registration from a California-approved organization. Additionally, 1-2 years of experience in a behavioral health setting is required.

The Case Manager is a clinical professional who manages the patients' cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to patients. The Case Manager provides quality care through developing, implementing, managing and evaluating patient care plans.
Compensation for roles at Fashion Valley CTC varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience. As required by local law, Fashion Valley CTC provides a reasonable hourly range of compensation for roles that may be hired in California as set forth below.Role Location: California
Calculated Salary Range for role: $29.14 - $38.00 per hour.
