JOB SUMMARY

Under the supervision of the Director of Housing and Outreach, the Client Care Assistant supports Case Managers by assisting clients in accessing housing, behavioral health, medical, and substance use treatment services. The CCA provides engagement, service coordination, appointment support, documentation assistance, crisis observation, and daily client follow-up while promoting housing stability and recovery. The position works closely with shelter staff, Housing Support Case Managers, DMHAS staff, healthcare providers, and community agencies to ensure continuity of care. This role is also inclusive of completing client intakes , orientation , biweekly documentation and maintaining a case load.


JOB DUTIES

1. Embodies Eva’s Village’s core values, voice and tone in order to provide an exceptional and effective experience for our clients, funders, employees and volunteers

2. Develops a thorough working knowledge of policies and procedures, with emphasis on funder requirements and licensure regulations as related to client care.

3. Develops a weekly schedule to ensure that incident reports, medication monitoring, grievance procedures are followed and audited weekly.

4. Makes recommendations to House Manager on training needed.

5. Responsible for training staff on procedures and monitoring compliance related to client care: incident reports, trauma-informed approach, medication monitoring, grievance, and letter of agreement protocols.

6. Audits HMIS data collection from intake to discharge monthly.

7. Runs monthly Data Quality Reports and resolves errors.

8. Makes recommendations to House Manager on additional HMIS Users and trains them accordingly.

9. Reviews the archiving procedure with staff and assigns staff to client chart archiving.

10. Follows Client Care process through the Case Management system, keeping accurate status reports of clients’ progress within Discharge Plan.

11. Meets with each client every other week (or weekly as needed) to assess progress, provide referrals and linkages.

12. Maintain regular communication with assigned Case Managers.

13. Assist with scheduling client appointments.

14. Prepare client files for appointments.

15. Track required documentation.

16. Monitor client progress toward service plan goals.

17. Escorts clients to and from appointments as needed, via walking or use of Company vehicle.

18. Attends Coordinated Assessment Meetings of the PCICH to advocate for housing opportunities for shelter clients.

19. Participates in weekly one to one meeting with the House Manager and EVP (as required) as well as staff meetings.

20. Organizes and runs bi-weekly house meetings.

21. Tracks outcomes of referrals to services

22. Assisting with obtaining identification, birth certificates, Social Security cards, and income verification.

23. Maintains a housing log of all clients who are deemed housing ready and assist clients navigating resources to become housing ready.

24. Reviews all Operational intakes for completion and accuracy and identify which case manager or shelter program clients should be assigned to with oversight of director of housing and outreach.

25. Coordinating detox, inpatient, outpatient, IOP, and MAT appointments.

26. Assist client’s with daily living support such as Obtaining clothing, laundry resources, Hygiene supplies and Food resources.

27. Responsible for facility recreational activities and holiday event planning or collaboration.

28. Any other duties assigned by immediate supervisor and/or EVP.


QUALIFICATIONS


EDUCATION: 

  • High School Diploma

EXPERIENCE: 

  • 3 to 5 years in residential services.
  • Experience with case management and assisting vulnerable populations.
  • Knowledge of Housing and Homelessness systems such as Coordinated Entry process, HMIS and HUD/ COC regulations.
  • Knowledge of the Housing first Model, diversion and prevention strategies and length of stay management.
  • Experience managing a caseload of 20 plus clients.
  • Experience providing Intensive case management to homeless or transient populations.
