Job Summary: The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This position is a full-time, day-shift role, where the provider would work 3-10 hour shifts from 8:00am-7:00pm. The provider will work collaboratively with physicians, nurses, and other healthcare professionals to deliver patient-centered care in a community health setting. Responsibilities include conducting comprehensive health assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans. The NP/PA will provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention. This role also involves managing follow-up care, coordinating referrals to specialists when necessary, and ensuring continuity of care for patients.
The provider will maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives. Strong communication skills are essential for educating patients about their health, collaborating with interdisciplinary teams, and fostering trust within the community. The NP/PA will also play a key role in addressing social determinants of health by connecting patients to appropriate resources and support services. This position requires a commitment to compassionate, culturally competent care and the ability to work effectively in a fast-paced environment while prioritizing patient needs.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices patient population. Participates in the practices quality improvement process and performance evaluation.
Education: Required: RN with a master's degree in nursing certification in family and/or adult nurse practitioner. Graduate of a nationally accredited school of nursing
Experience: Required: Previous experience in a FQHC strongly preferred
Position Requirement(s): License/Certification/Registration Required: Current licensure to practice nursing in the State of Tennessee. Eligibility to sit for certification in area of specialty
The Nurse Licensure Compact will not change how to obtain or renew a Tennessee license. However, the Tennessee nursing license will be a single state license for Tennessee Residents or non-compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.

Department Position Summary: A nurse practitioner in PH Family Practice/Internal Medicine will work under direction of the physicians/medical director. He/She will be responsible for developing evidence-based protocols and utilizing them to develop an individualized plan of care. He/She must be able to collect, document, and analyze patient data. They must be able to collaborate with the health care team in the development and implementation of testing and treatment according to protocols. He/She must be able to participate in development and implementation health screening program for adolescents and adults and develop protocols for identification of high-risk individuals. He/She must participate in community and corporate education on internal medicine topics.
The individual must demonstrate knowledge of the principles of growth and development over their life span and possess the ability to assess data reflective of the patients status. He/She must interpret the appropriate information needed to identify each patients requirements relative to his or her age-specific needs and to provide the care needed as described in the units policies and procedures. This is done through evaluation of direct supervisor by direct observation, in a skills lab, or discussed verbally or written. Competency can also be evaluated by a formal peer review process and/or by self-assessment based on objective criteria that is reviewed and validated.
