The LPN Team Lead oversees a team of embedded care coordinators while maintaining a clinical presence at dialysis clinics to ensure high-quality patient care. Responsibilities include managing daily clinical tasks, monitoring team performance against KPIs, and fostering strong relationships with facility staff.
Candidates must hold a valid LPN or CMA license and possess at least two years of healthcare management experience. A valid driver's license and access to a vehicle are required for travel between clinic locations.

The LPN Care Coordinator Team Lead is a senior, field-based role within Ennoble Care’s Care Coordination program. This position combines hands-on, embedded care coordination responsibilities with the oversight, leadership, and operational management of a team of 8–12 Embedded Care Coordinators (CCs) across clinics.
The Team Lead serves as the subject matter expert for facility-based care coordination, acting as a trusted bridge between Ennoble Care’s clinical teams, clinical staff, and the broader care coordination function.
Ennoble Care fosters an integrated, respectful culture that values accountability, meaningful impact, and long-term professional growth based on performance and organizational needs.
