The role involves coordinating post-acute care transitions to improve patient outcomes and referral source satisfaction. It requires managing strategic relationships with health systems, hospitals, and physicians to promote home health services.
Candidates must be a licensed RN or PT with at least two years of field experience and knowledge of Medicare regulatory guidelines. A valid driver's license and dependable transportation are mandatory.
