The Case Manager coordinates patient care from admission to discharge by facilitating interdisciplinary team conferences and managing treatment plans. They are responsible for monitoring patient progress, ensuring regulatory compliance, and educating families on the rehabilitation process.
Candidates must hold an active professional license in their discipline, such as RN, SW, OT, PT, or ST. A minimum of an associate degree for nursing or a bachelor's degree for other disciplines is required, with CCM or ACM certification expected within two years.
