The Physical Therapist provides skilled therapy services, including evaluations and individualized treatment plans, to patients in their homes. They collaborate with an interdisciplinary team to improve patient mobility, safety, and independence while supervising assistants and aides.
Candidates must be graduates of an accredited Physical Therapy program with a valid state license and CPR certification. A minimum of one year of experience in physical therapy is required, along with a valid driver's license and reliable transportation.

The Physical Therapist (PT) provides skilled physical therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations.
The PT evaluates patient mobility, strength, balance, and functional status, develops and implements therapy interventions, and works collaboratively with the interdisciplinary care team to improve safety, independence, and quality of life.
This position reports to the Director of Nursing (DON), Clinical Manager, or designated clinical leadership. The PT functions as part of the interdisciplinary care team and provides supervision to the Physical Therapist Assistant (PTA) and Home Health Aide when required by the patient’s plan of care and agency policy.
The Physical Therapist performs services within the scope of Florida PT licensure, Medicare Conditions of Participation, physician orders, and agency policies. The PT is responsible for evaluation, treatment planning, reassessment, discharge planning, and therapy supervision within the home health setting.
Work is performed in patient homes and community settings with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.
