The Admission Nurse is responsible for conducting initial evaluations and assessments of hospice patients to identify physical, psychosocial, and environmental needs. They implement and revise plans of care while communicating critical changes in patient condition to the medical team.
Candidates must be a graduate of an accredited nursing program with a current, unrestricted state RN license and at least one year of clinical care experience. A valid driver's license, dependable vehicle, and the ability to travel 50% of the time are required.

Admission Nurse- 5K SIGN ON BONUS
We are looking for someone to work four 10-hour shifts each week on Friday, Saturday, Sunday, and Monday. Friday, Saturday, and Sunday are required, but we can offer flexibility on the fourth shift if a candidate would prefer to work a different weekday instead of Monday.

What is Required?
You will report to the Clinical Team Manager or Branch Director.
The base salary for this position is $83,000 to $95,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
