The Registered Nurse Case Manager assesses the physical, psychosocial, and environmental needs of hospice patients and their families. They coordinate care through an Interdisciplinary Plan of Care and provide essential support and education to patients, families, and facility staff.
Candidates must be a graduate of an accredited nursing program with a current State RN license and at least one year of clinical care experience. A valid driver's license, reliable transportation, and the ability to travel 50% of the time are required.
