Provide high-quality hospice care to patients in their homes according to the established Plan of Care and physician orders. Communicate patient condition changes to the RN Case Manager and implement safety plans to prevent avoidable hospitalizations.
Requires a High School Diploma or GED and at least one year of experience as a Hospice Aide or Nursing Assistant. Must complete a state-established Hospice Aide training program and possess a valid driver's license with reliable transportation.
