The Medical Social Worker assesses patients' psychosocial, financial, and environmental needs to develop and implement a comprehensive Plan of Care. They provide counseling and coordinate with physicians and families to ensure quality continuity of care and safe discharge planning.
Requires a Master's or Doctoral degree in Social Work from a CSWE-accredited school and a current State License as a Social Worker. Candidates must have at least one year of healthcare social work experience and a valid driver's license for required travel.
