The Hospice CNA/HHA provides high-quality patient care in home settings according to the established Plan of Care and physician orders. They are responsible for documenting patient status, communicating changes in condition to the RN Case Manager, and implementing safety plans to prevent avoidable hospitalizations.
Candidates must have a High School Diploma or GED and at least one year of experience as a Hospice Aide or Nursing Assistant. A state-established Hospice Aide training program certification and a valid driver's license with reliable transportation are required.
