The LPN provides high-quality nursing care to patients in their homes according to established plans of care and physician orders. They are responsible for monitoring patient conditions, documenting medication regimens, and collaborating with RN Case Managers to prevent avoidable hospitalizations.
Candidates must be a graduate of an accredited nursing school with a current State License as an LPN/LVN and at least one year of clinical experience. A valid driver's license, reliable transportation, and the ability to lift up to 50 pounds are required.

You will report to the Clinical Team Manager.
The base salary for this position is $30 to $35 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
