JOB PURPOSE: Collaborates with clinical team members to facilitate unmet health care needs of our patients and their families. This occurs through health promotion and health education activities for individuals and groups, outreach activities in the community, coordinating group visits in the clinic settings, receiving referrals from clinical team members for addressing basic needs and advocacy, reinforcing needed education and self-management skills, coordinating care management with other clinical team members, and documenting in the EMR.
The primary goal of the El Rio Health Clinic Health Advisor is to support El Rio’s Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions.
