Conduct thorough aged care assessments to identify the needs of older adults and facilitate referrals to essential services. Promote the retention of function and capability in daily living while ensuring compliance with Department of Health requirements.
Requires at least one year of experience working with older Australians and tertiary education in a health-related discipline with professional registration (e.g., AHPRA). Must possess a current Australian driver's licence and a comprehensively insured vehicle for daily travel.
As a Clinical Aged Care Assessor, you'll play a crucial role in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life.
Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most
