Conduct thorough aged care assessments to identify the needs of older adults and facilitate referrals to essential services. Work closely with families and carers to promote independent living and ensure compliance with Department of Health requirements.
Requires at least one year of experience in aged care and a tertiary degree in a health-related discipline. Must hold current AHPRA registration, a valid Australian driver's licence, and a comprehensively insured vehicle.
As a Clinical Aged Care Assessor, you'll play a crucial role in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life.
Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most
