Act as a bridge between community members and the healthcare system through outreach and education to improve health outcomes. Provide support care to socially and medically complex patients in non-traditional settings while collaborating with healthcare providers.
Requires a High School Diploma or GED and 1-3 years of experience in community outreach or home visits, specifically with Medicaid populations. Must have reliable transportation and be willing to travel 50-75% of the time within a 90-mile radius of Macon or Sagamon County.

Scheduled hours: M-F from 8:00am to 5:00pm
This is a field-based position working with the members/community in Macon or Sagamon County and surrounding counties.
Community Health Workers (CHW) serve the community as a bridge between the member (community) and the health care system (providers) through outreach and education. By providing these services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties become more acculturated with each other.
This position will work under the direction of a designated professional.

40The typical pay range for this role is:
$18.50 - $38.82This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
