The Patient Support Case Manager serves as the primary point of contact for patients, caregivers, and providers to facilitate the patient journey and ensure treatment access. They manage inbound and outbound communications, document patient progress, and provide educational resources while navigating insurance and pharmacy processes.
Candidates must have a minimum of 5 years of pharmacy experience. A background in patient support programs and relevant certifications such as PACS, CHES, or CCM are preferred.
