Provide family medicine care within an integrated healthcare system at the County Line Family Medicine Practice. Transition from academia into clinical practice through a systematic fellowship training program.
Must hold or successfully complete a nationally recognized Advanced Practice Nurse certification. Must be eligible for prescriptive authority under the Indiana State Board of Nursing criteria.

This opening is Advanced Practice Provider Fellowship Program eligible! Community Health Network has developed a holistic program to assist newly graduated Advanced Practice Provider. The goal of this intentional and systematic training program is to support the transition of new providers from academia into clinical practice and prepare them for long careers within Community Health Network. Your journey with the Advanced Practice Provider Fellowship Program begins once you have accepted a role with our Network.

Outpatient only, no on-call or weekend requirements
