The technician is responsible for obtaining and documenting accurate medication histories in the EPIC electronic medical record. This involves interviewing patients and families and coordinating with outpatient pharmacies and long-term care facilities.
Candidates must have a high school diploma or GED and be licensed as a Pharmacy Technician or Pharmacy Intern by the IPLA. National certification through PTCB or NHA is required within two years of hire for technicians.

We are looking for Pharmacist students for our paid Medication Reconciliation Technician positions. Flexible schedules for students. The Medication History Technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards.
Your exceptional skills
Interpret and enter “Prior to Admission” medication history into the EPIC electronic medical record, perform thorough interviews with patients and family members, conduct telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history, track interventions and productivity using department’s process, regularly and professionally communicate with patients, their families, and hospital staff

High School diploma or GED equivalent required.
Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA)
National Certification through the PTCB or NHA required within two years of hire
