The Navigator II identifies and reduces barriers for clients by connecting them with essential services and resources. They are responsible for outreach, case management, and providing wrap-around supportive services to vulnerable populations.
A high school diploma or GED is required, and at least 90 days of experience as a Navigator or Peer Support Specialist is mandatory. Candidates must have a minimum of 12 months of recovery from substance use or mental health disorders.

CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.
Job Summary
The Navigator II’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator II will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team made up of internal and external stakeholders. The Navigator II will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.
In this role, you will be on the front lines of delivering lifesaving support to vulnerable populations affected by extreme heat. You will collaborate with community members, local service providers, and internal teams to distribute resources, share information, and connect people in need with the appropriate services. This position requires a strong commitment to community engagement, empathy, and responsiveness to the urgent needs of individuals experiencing or at risk of heat-related health issues.
