The Social Services Coordinator assesses resident needs, develops individualized care plans, and provides emotional and psychological support during life transitions. They also collaborate with the interdisciplinary team to ensure regulatory compliance and advocate for the dignity and independence of residents.
A Bachelor’s degree in Social Services or a related field is required, with a Master’s degree preferred. Candidates should have at least three years of experience in a skilled nursing facility and possess strong knowledge of federal and state regulations.

Full Time Position: Monday - Friday 8:30am-5pm
Salary starts at $20/hour
