Perform comprehensive physical therapy evaluations and develop individualized, evidence-based plans of care to improve patient mobility and independence. Collaborate with an interdisciplinary team and educate patients and caregivers on safe functional movement and home exercise programs.
Candidates must possess a current Florida Physical Therapist (PT) license, a valid driver's license, and reliable transportation. Home health experience and familiarity with WellSky EMR are preferred but not required.
