The Nurse Practitioner will provide comprehensive, personalized primary care to seniors while collaborating with a multidisciplinary team. Responsibilities include managing patient referrals, coordinating home health services, and documenting care according to clinical standards.
Candidates must hold a Master's Degree in Nursing or have completed a PA program with board certification. An active, unrestricted NP or PA license in the state of practice is required.
As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.
