Provide skilled nursing care in participants' homes and coordinate with an interdisciplinary team to develop individualized care plans. Perform comprehensive nursing assessments, administer treatments, and educate participants on disease management to prevent avoidable hospitalizations.
Requires a current New York State Registered Nurse license and at least one year of experience in home health, acute care, or critical care settings. Candidates must possess a valid driver's license, reliable vehicle, and strong clinical documentation skills.

Eligibility for additional compensation based on verified nursing experience and qualifying nursing education in accordance with the NYSNA Collective Bargaining Agreement.
At CenterLight Healthcare PACE, you'll build lasting relationships with older adults while helping them remain safe, healthy, and independent in their own homes. Unlike traditional home care, you'll be part of an Interdisciplinary Team (IDT) where your clinical expertise directly shapes each participant's individualized care plan.
PACE nursing allows you to practice at the top of your license while developing lasting relationships with participants.
You'll collaborate with physicians, therapists, social workers, dietitians and other healthcare professionals to deliver coordinated, person-centered care that helps older adults remain safely in their homes.
