DUTY STATEMENT AND RESPONSIBILITIES

Job Summary:

Under the supervision of the Program Coordinator, will assist in providing residential program services. Will cooperate with other staff personnel in the planning, evaluation, training, and implementation of a process for program efficiency and client care:

Job Functions:

  • Coordinate with ancillary services, including individualized connection, referral, and linkages to community-based services and supports, including but not limited to educational, social, prevocational, vocational, housing, nutritional, criminal justice, transportation, childcare, child development, family/marriage education, cultural sources, and mutual aid support groups.
  • Discharge planning, including coordinating with SUD treatment providers to support transitions between levels of care and recovery resources, referrals to mental health providers, and referrals to primary or specialty medical providers.
  • Coordinate with medical and mental health care providers to monitor and support comorbid health conditions.
  • Document care coordination notes in EHR in 15-minute increments
  • Collaborate with other professionals within the agency to meet the needs of the beneficiary.
  • Collaborate with other professionals outside of the agency, such as San Bernardino County Care Coordinators, Riverside County Substance Use Community Action Resource Evaluation Services Case Managers, Mental Health Providers, Primary Care Physicians, and Medicated Assisted Treatment Providers, to address the beneficiary's needs.
  • Arranging transportation services to and from appointments.
  • Provide quality service to clients as required by Cedar House Life Change Center, State, and County Requirements. 
  • Maintain timely and reliable attendance.
  • Safeguard resident's and visitor’s welfare at all times.
  • Pursue additional status/level advancement whenever possible.
  • Perform life-saving measures when needed to protect the lives of Cedar House clients and employees. 
  • Consistently applies trauma-responsive practices to promote safety, trust, and healing.    
  • Required to train or assist in the trainings of co-workers as needed.
  • Other duties and/or responsibilities as assigned.

Requirements

QUALIFICATIONS

Experience:

  • Be certified upon hire under an accredited drug and alcohol certification program, CAADE, CADTP, or CCAPP (preferred).
  • Be registered upon hire under an accredited drug and alcohol certification program, CAADE, CADTP, or CCAPP (required).
  • Have at least 1 year of experience working in a residential setting.

Education:

  • Complete continuing education and trainings as required by Cedar House, County, and State mandates.
  • Completed twelve years of education or have a GED.
  • Maintain current CPR/First Aid/AED.

Knowledge/Skills/Abilities:

  • Knowledgeable of Confidentiality laws, policies, and procedures.
  • Resourcefulness, creativity, and strong problem-solving skills.
  • Always enforce all house rules and regulations.
  • Continue to acquire and update the knowledge and skills necessary to address chemical dependency and related counseling issues.
  • Maintain a professional demeanor at all times.
  • Ability to address stressful situations in a calm manner and be able to communicate in a positive way.
  • Adhere to all policies and procedures of the organization.
  • Ability to work with diverse groups of people.

PHYSICAL REQUIREMENTS

The physical requirements described here represent those that an employee must meet to perform the essential functions of this job successfully. Reasonable accommodations may be made to enable Individuals with disabilities to perform these functions.

While performing the duties of this job, the employee must be able to remain in a stationary position 80% of the time. They will also need to move around inside the office occasionally to access filing cabinets, office machinery, etc. The employee must be capable of operating a computer and other office productivity machinery. There may be occasions where the employee needs to climb up or down a ladder or equipment and position themselves to maintain files in file cabinets. It is essential for the employee to communicate information and ideas clearly so that others can understand. They must also be able to provide accurate information in these situations. The employee should be able to observe details at close range and occasionally move boxes weighing up to 20 pounds for various reasons. Additionally, the employee may need to work around or be exposed to outdoor weather conditions.
