Position Summary

The RN Unit Manager is responsible for providing and overseeing the delivery of high-quality nursing care to residents in accordance with physician orders, individualized care plans, facility policies, and all applicable federal, state, and local regulations. The Unit Manager provides clinical leadership and supervision for licensed nurses, certified nursing assistants, medication aides, and other nursing personnel while promoting resident-centered care, regulatory compliance, quality outcomes, and interdisciplinary collaboration.

Essential Duties and Responsibilities

  • Provide nursing care to residents as prescribed by the attending physician and in accordance with established facility policies, accepted standards of nursing practice, and applicable federal, state, and local regulations.
  • Lead, direct, supervise, and support licensed nurses, certified nursing assistants, medication aides, and other professional and non-professional staff in the delivery of resident care.
  • Conduct resident rounds at least twice daily to assess resident condition, identify clinical concerns, and ensure quality care is being provided.
  • Assess the overall status of residents and accurately document findings in the medical record as indicated.
  • Monitor resident conditions and provide nursing care in accordance with physician orders, individualized care plans, facility policies, and regulatory requirements.
  • Coordinate, prepare for, and assist with resident admissions, transfers, discharges, and readmissions to ensure continuity of care.
  • Communicate effectively with residents, families, physicians, interdisciplinary team members, consultants, hospitals, pharmacies, and other healthcare professionals regarding resident status, treatment plans, and changes in condition.
  • Lead, supervise, monitor, and support licensed personnel and other nursing staff to ensure safe, effective, and compassionate resident care.
  • Schedule daily job assignments, staffing responsibilities, and nursing duties to ensure appropriate resident coverage.
  • Supervise, evaluate, coach, mentor, and recommend disciplinary action regarding the performance of nursing personnel in accordance with facility policies.
  • Assist with the planning, coordination, and presentation of orientation programs, in-service education, competency validation, and continuing education for nursing staff.
  • Monitor the care environment and staff practices to ensure compliance with fire safety, disaster preparedness, infection prevention, bloodborne pathogen standards, OSHA requirements, and all facility safety programs.
  • Assist in the development, implementation, review, and revision of individualized resident care plans in collaboration with the interdisciplinary team.
  • Delegate nursing responsibilities appropriately based upon staff qualifications, competency, resident acuity, and scope of practice.
  • Complete or assist with performance evaluations of nursing personnel.
  • Report occupational exposures, injuries, and workplace incidents in accordance with facility policy and OSHA regulations.
  • Make daily rounds to observe nursing staff performing assigned duties and provide coaching, guidance, and corrective action as necessary to ensure compliance with facility policies and standards of practice.
  • Demonstrate knowledge of and actively promote resident rights, dignity, privacy, confidentiality, and person-centered care.
  • Recognize, prevent, and immediately report suspected abuse, neglect, exploitation, or mistreatment of residents in accordance with facility policies and applicable regulations.
  • Promote and maintain a safe, clean, organized, and therapeutic environment for residents, visitors, and staff.
  • Review the Twenty-Four Hour Report daily to remain informed of changes in resident condition, physician orders, new admissions, incidents, and treatment plans.
  • Participate in the Minimum Data Set (MDS) process, Resident Assessment Instrument (RAI), interdisciplinary care plan meetings, Medicare meetings, and Total Care Plan conferences as indicated.
  • Obtain laboratory specimens as ordered or ensure laboratory services are completed in accordance with physician orders and facility policies.
  • Monitor and track laboratory orders and results, promptly communicating abnormal findings to the physician and implementing follow-up interventions as appropriate.
  • Provide direct nursing care and assist with floor nursing responsibilities as needed to ensure resident needs are met.
  • Complete weekly Medicare progress notes accurately and within required timeframes.
  • Attend and actively participate in Quality Assurance and Performance Improvement (QAPI), Infection Prevention, Safety, Risk Management, Utilization Review, and other required committee meetings.
  • Serve as an active member of the interdisciplinary team by placing resident safety, dignity, quality of life, and clinical outcomes as the highest priority.
  • Review, monitor, and document weekly resident weights, identifying significant weight changes and communicating concerns to the physician, Registered Dietitian, and interdisciplinary team.
  • Maintain accurate, timely, and complete clinical documentation in accordance with CMS regulations, facility policy, and accepted nursing standards.
  • Ensure compliance with HIPAA, resident confidentiality requirements, and all facility policies and procedures.
  • Assist with survey preparation, regulatory compliance, mock surveys, implementation of Plans of Correction, and ongoing quality improvement initiatives.

 

Treatment Nurse

Description

To provide nursing care to the residents as prescribed by the physician, and in accordance with established facility protocol, local, state, and federal regulations.

Requirements

  • Follow established standards of nursing practice to implement facility policies and procedures related to treatment and medication administration
  • Interpret existing policies and procedures to nursing assistants, residents, families, and physicians
  • Supervise and evaluate all direct resident care provided and initiate appropriate action as necessary.  Provide clinical supervision to CNAs
  • Conduct resident rounds as assigned to access the skin conditions of each resident and report problems to charge nurse and take necessary follow-up
  • Identify safety hazards and initiate corrective action
  • Compile information for facility committees as instructed
  • Administer treatments and medications according to the physician's order and accurately record treatment and medications administered
  • Document resident care provided and resident's response or lack of response to treatment or medication provided
  • Assess each resident's skin condition weekly, and as necessary, including stage, size, sites, depth, color, drainage and odor of each condition
  • Obtain physicians' orders and any revisions for treatment or medications as necessary
  • Perform other duties as directed by supervisor
  • Perform all other duties and responsibilities as assigned by the Director of Nursing, Administrator, or designee.

Touch: In order to detect changes in temperature

LICENSURE REQUIREMENTS

Must be a Registered Nurse (R.N.) currently licensed and in good standing with the Board of Nursing in the State in which the facility is located.

SUMMARY OF OCCUPATIONAL EXPOSURE

Tasks and procedures performed by the employee involve risks classified by C.D.C. as Category I. (Direct contact with blood or other bodily fluids to which universal precautions apply.)

Other Considerations and Requirements:

Must be able to tolerate a high pace as typical for a nursing facility. Must be able to take and give instructions well and enjoy working with the elderly. Although employee is able to sit, he/she must also be able to tolerate prolonged standing and walking.
