Manage patient census to close clinical and non-clinical care gaps while providing education on chronic condition management. Coordinate community resources, medication refills, and referrals to ensure timely delivery of healthcare services.
Requires an active New York LPN license and at least 3 years of direct patient-facing experience. Candidates must possess technical aptitude in Microsoft Office and have a HIPAA-compliant home office with high-speed Wi-Fi.
