Deliver basic needs support services to child welfare-involved families and coordinate patient outcomes through the Community Care Hub. Conduct outreach in various healthcare and community settings while managing referrals and maintaining client documentation.
Requires experience in care coordination and proficiency with technology and client management systems. A Community Health Worker certificate and lived experience with underserved or rural populations are preferred.

About the Position
The Referral and Care Coordination Specialist is a temporary position through July 31, 2027 to support client care and referral efforts for the Community Care Hub. Responsibilities include:
