The RN Case Manager performs patient assessments and collaborates with the care team to develop and implement individualized plans of care. They also supervise LPNs and HHAs while promoting patient health through education and rehabilitative measures.
Candidates must have a current RN license and at least one year of clinical experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
