Conduct home visits to perform diagnostic tests, measurements, and safety assessments for adult and geriatric clients. Develop and implement individualized care plans including therapeutic exercises and gait training while documenting all interventions via tablet.
Requires a current Pennsylvania Physical Therapist license and at least one year of recent work experience in physical therapy. Must be a graduate of an APTA or AMA approved program and possess strong organizational and computer skills.
