Provide 1:1 skilled nursing and supportive care to patients in their homes, focusing on wound, ostomy, and continence needs. Conduct consultations, perform assessments, and provide education to patients and their families to ensure a safe transition from hospital to home.
Requires an active PA Registered Nurse (RN) license and a Wound, Ostomy and Continence Nurse (WOCN) certification. Candidates must have at least one year of nursing experience and the ability to travel to assigned cases.
