Support Coordinator –
The Support Coordinator is responsible for coordinating and monitoring services for adults with intellectual and developmental disabilities enrolled in New Jersey's Division of Developmental Disabilities (DDD) programs. This position focuses on person-centered planning, advocacy, and connecting individuals to the supports and resources needed to live safely and independently within their communities. Support Coordinators maintain regular contact with individuals, complete annual and ongoing assessments, develop and update Individualized Service Plans (ISPs), monitor service delivery, and ensure compliance with DDD and Medicaid waiver requirements. They collaborate with families, providers, medical professionals, and interdisciplinary teams while promoting each individual's goals, rights, health, safety, and quality of life. The role requires strong organizational, communication, documentation, and problem-solving skills, along with the ability to effectively manage a diverse caseload in a fast-paced environment.
Key Responsibilities
Develop, implement, and maintain person-centered Individualized Service Plans (ISPs).
Conduct monthly monitoring and maintain regular communication with individuals and their support teams.
Coordinate DDD-funded services and community resources.
Advocate for individuals' health, safety, rights, and personal goals.
Monitor provider performance and service effectiveness.
Complete timely documentation in accordance with DDD, Medicaid, and agency requirements.
Coordinate crisis intervention and assist with problem resolution when needed.
Collaborate with families, guardians, healthcare providers, and community agencies.
Maintain compliance with all DDD policies, regulations, and documentation standards.
Manage a caseload while meeting required deadlines and performance expectations.
Preferred Qualifications
Bachelor's degree in Social Work, Psychology, Human Services, Sociology, Special Education, or a related field.
Experience working with individuals with intellectual and developmental disabilities
Knowledge of NJ DDD programs, Medicaid waiver services, and person-centered planning is a plus.
Excellent communication, organizational, and time-management skills.
Ability to work independently while collaborating with multidisciplinary teams.
Valid driver's license and reliable transportation.
