Candidates must hold an unrestricted NP/PA-C certification and a valid medical license and DEA certification in Indiana. Preference is given to those with at least one year of experience in substance use disorder, psychiatry, or mental health.

Avenues Recovery Center at Fort Wayne is a 100- bed residential facility offering drug and alcohol detox and inpatient rehabilitation services.
