Execute nursing interventions and carry out physician orders in strict compliance with organizational standards and clinical protocols.
Develop, implement, and track individualized patient care plans while documenting and reporting status changes in real time.
Serve as the primary point of contact across all healthcare disciplines to ensure cohesive, well-integrated patient care.
Evaluate post-care needs and collaborate with the healthcare team to facilitate smooth patient transitions and discharge continuity.
