About the Role:
As a Practitioner, Child and Youth, you will provide early intervention and prevention services to children, young people, and their parents, carers, and significant others. Through a combination of case management, counselling, advocacy, mediation, and practical support, you will empower children and young people, particularly those at risk—to remain connected with their families, education, training, employment, and their communities.
You will work collaboratively with internal and external stakeholders, undertaking extensive networking, community capacity building, and research to deliver positive outcomes for children, young people, and families in line with funding and contractual requirements.
This is a fixed-term, part-time (0.8 FTE) position based in Toowoomba until 30th of June 2027. The salary for this position starts from $46.70 per hour, plus superannuation and access to salary packaging benefits.
Responsibilities:
Provide trauma-informed case management, counselling, advocacy, mediation, and practical support to children, young people, and their families.
Complete holistic assessments, develop individual support plans, and monitor client progress using outcomes-based approaches.
Conduct risk assessments and develop safety plans to promote the wellbeing of children and young people.
Support children and young people to strengthen connections with family, education, training, employment, and their local community.
Deliver information, referrals, and coordinated support by working collaboratively with schools, community organisations, and government services.
Facilitate individual, family, and group programs that build resilience, social skills, mental wellbeing, and positive relationships.
Conduct outreach services, including school visits, home visits, and community-based support, in line with service requirements.
Maintain accurate case records, manage caseloads effectively, and ensure compliance with legislative, contractual, and quality standards.
Build and maintain strong partnerships with child protection, youth justice, health, education, and other key stakeholders to achieve positive client outcomes.
Contribute to continuous service improvement by participating in team meetings, professional development, supervision, data collection, and quality improvement initiatives.
Qualifications, Credentials and Other Requirements
Degree in Social Work, Psychology, Human Services, Behavioural Science or similar field
Current National Police Certificate
Current QLD Drivers’ Licence & own reliable vehicle
Current Blue Card
Willingness to travel within service region
