The role involves coordinating the transition of patients from acute care to home health by gathering medical history and arranging start-of-care visits. Responsibilities also include providing bedside education, ordering medical equipment, and conducting community outreach to referral sources.
Candidates must hold a valid RN license in good standing. A minimum of one year of experience in both an acute care setting and a customer service capacity is required.
