The Social Worker is responsible for conducting psychosocial evaluations and providing ongoing counseling to patients and families during end-of-life care. They provide emotional support and work in accordance with a designated plan of care to ensure meaningful experiences.
Candidates must hold an MSW from an accredited school and a current state social work license. At least 2 years of clinical experience, preferably in healthcare or hospice, and reliable transportation are required.

Working in accordance with the plan of care, you will provide emotional support to patients and families when it's needed most.
