The RN Care Manager assesses participant care needs, provides nursing interventions, and develops comprehensive care plans in collaboration with an interdisciplinary team. They coordinate home care services and monitor acute and chronic needs across various settings including clinics and homes.
Requires a valid State RN License, BLS certification, and at least one year of experience providing care as an RN. A Bachelor of Science in Nursing and experience with elderly or frail populations are preferred.

BoldAge PACE is an all-inclusive program of care, personalized to meet the individual health and well-being needs of our participants. Our approach is simple: We listen to our participants and their caregivers to truly understand their needs and desires.
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants’ needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
