Summary:

As a part of the Goldbelt Glacier Team, you will be supporting the Naval Logistics Command with medical services for active-duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries designated by the Government. As a HealthCare Worker (HCW), you will be assigned to either a Military Treatment Facility (MTF) or other Federal medical treatment facilities in Japan.

Responsibilities

Essential Job Functions:

  

• The HCW is required to perform fourteen - seventeen (14 - 17) shifts of twelve - sixteen (12 - 16) hours each per monthly schedule. The daytime shifts (starts at 0545-1800) will alternate quarterly with night shift (starts at 1745-0600). If the HCW cannot report to work as scheduled due to an emergency or illness, the HCW shall promptly notify the supervisor and charge nurse for resolution. No shows or sudden disappearances may be reported to the contracting office and may resolve in negative reports against the company and could result in the termination process in accordance with FAR 12.403. The HCW may be required to work various shifts or augment clinical nursing support during the absence of other team nurses due to illness, leave, or deployment. The HCW may be required to work more than 12 hours but no greater than 16 hours in a single shift, as determined by the supervisor or designee based on the needs of the MTF. The current method of communicating schedules is effective. Sufficient notice is provided, and schedules are clearly presented in two locations: a physical in-office posting and through the Signal group chat.
• Operating Instruction (OI). The HCW must read and understand OI 46-06 which can be found in the unit. Not all of the information applies to contractor but HCW must read when the OI is updated. If there are any questions, they must ask, or the government would understand the HCW understand all OI term and condition.
• Shift release does NOT apply to HCW. They must work as scheduled.
• The government start utilizing own timesheet to track contractor’s work hours. The approved timesheet must be distributed by contractor for their employee. The Person in Charge (PIC) for the day/shift must sign every time the contractor leaves for the day.
• The HCW will be expected to perform on-call duties to support mission. These duties include a 24 hour call every other week, call will be taken from home beginning at 0600 and ending at 0600 the following day. If called into work, the HCW must arrive within 30 minutes of phone call. Due to on call requirements, the HCW will live in a location that will allow for the allotted response time. The HCW will not work greater than 16 hours in a 24 hour period if called in.
• Contractor will get paid to be on call and stand by at home and then paid an additional amount if called into work.
• As part of the HCW’s normal tour of duty, service will be required on the day of observance of Federal holidays and Non-Duty Days/Closures.
• Due to the 24-hour operation of the Labor and Delivery Unit, if the commander determines the base is closed due to snow, hurricane, or acts of God for non-essential personnel, the unit remains open and the HCW is still expected to show for work unless physically unable to travel to the assigned duty location. If unable to make it to work, the HCW shall annotate their time sheet as such and notify the supervisor and charge nurse that they are unable to report to work. Such occurrences are not billable to the government. The contractor shall only invoice for actual hours worked. Under no circumstance will the government be liable for hours not worked as a result of base closure or excusal from duty by the commander, regardless of the reason (emergency or otherwise).
• PTO will be approved on a case-by-case basis and will be granted at the discretion of the supervisor and must be in accordance with manning and mission requirements. In order to ensure that patient’s access to care is not limited due to manning, timing of leave shall be coordinated with supervisor as much as possible, at least 30 days in advance. Unscheduled absences shall be called into the FRED and charge nurse by the HCW at least two (2) hours prior to each duty day that he/she is unable to report to work. Unplanned absences resulting in lateness, regardless of the reason, employees can utilize PTO for only the time they are late, completing the remainder of their scheduled shift. This clarifies the previous understanding that any absence required a full 8 hours of PTO; now, PTO will only be used for the actual time missed due to lateness (for example, 2 hours PTO for a 2-hour delay, followed by 10 hours of work). In instances of unplanned leave, On-Call RN will be called if an On-Call RN is available.
• Any time exceeding a contractor’s scheduled start time will be considered lateness and charged as PTO.
• PTO will be counted as eight (8) hours each day. For pre-planned leave (such as vacations or out-oftown trips), 8-hour PTO increments will be used to ensure adequate coverage, as outlined in the OI. Any PTO request will be relayed to Contractor to verify the HCW has sufficient accrued leave time to cover the request. PTO requests exceeding 14 consecutive days will not be approved.
• PTO replaces scheduled shifts. If a contractor takes 2-3 days of PTO, all hours must be reflected on their timesheet as PTO, calculated at 8 hours per day. Therefore, 2-3 days of PTO will equate to 16-24 hours, representing the equivalent of two to three 8-hour shifts or two 12-hour shifts.
• If a HCW requests time off or PTO, is denied, and subsequently calls in sick in a pattern that appears related to the denied request, this must be documented immediately and reported to Contractor and the CS / KO ofthe contract.
• Contractors can request up to four Request Off (RO) days in a month, with a limit of one RO daybefore a leave period and one RO day after a leave period. However, they must not have 2 ROs in row before and after leave, and/or no more than 2 days off if it’s combined with a day off.
• PTO takes precedence over RO days, and RO days take precedence over scheduled days off. This means PTO requests are most likely to be honored, RO requests will likely be honored barring emergencies, and scheduled days off will be honored when operations are running smoothly.
• A doctor’s note will be required for sick leave verification when a HCW is absent for three or more consecutive shift days..
• The HCW will be enrolled for two weeks orientation in the unit, Monday through Friday, 0600-1400. The unit will be flexible with the orientation schedule day to day hours to ensure the HCW gets an adequate orientation to include 1 delivery, 1 discharge, 1 outpatient, and 1 triage. Additionally all orientation item checklists should be signed off by a charge nurse no later than at the completion of the 2 week orientation period. The HCW will have 1 month trial period (2 weeks orientation plus 2 weeks off orientation). During this trial period, the contractor will be evaluated by L&D and OBGYN staff based on job performance and patient safety standards to ensure the HCW is a good fit for the L&D unit. Should performance not be up to standard, Misawa AB reserves the right to request to the contractor to replace their HCW in the interest of patient safety at the end of the 1 month trial period. If they fail to provide qualified personnel twice in row, the government reserves the right to follow termination procedures as outlined in FAR 12.403 as the fact they are unable to fill in the position over 30 days.
• Coordinates patient care through a continuum and facilitates the achievement of high quality, safe and cost effective care.
• Participates in the orientation and training of newly assigned personnel, as appropriate.
• Ensures compliance with standards of care and practice in accordance with all established Unit Operating Instructions (which follow national guidance from AWHONN, American College of Obstetricians and Gynecologists (ACOG) and the American Academy of Pediatrics (AAP), Medical Group Instructions, and Air Force Instructions used in the medical treatment facility. Provides care within ethical and legal boundaries.
• Provides appropriate patient assessment skills.
• Accountable for making patient care assignments based on the scope of practice and skill level utilizing the nursing process as a basis for professional practice.
• Sets up, operates, and monitors specialized medical equipment such as IV infusion pumps, PCA pumps, vital sign machines, electronic fetal monitoring, cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, ultrasound, emergency transport and other unit-specific equipment.
• Recognizes adverse signs and symptoms and reacts quickly in emergency situations.
• Makes referral appointments and arranges specialty care, as applicable.
• Completes orientation and competency verification programs in accordance with unit guidelines.
• Serves and participates in committees, functions and other meetings as directed. Provides relevant and timely information to these groups, and assists with decision making and process improvement. Participates in customer service initiatives and medical readiness activities designed to enhance health services.
• Performs sterile vaginal examinations, speculum examinations, and diagnostic procedures via electronic fetal monitoring and tocometer.
• Prepares patients for surgery, ultrasound, amniocentesis, or other prescribed medical procedures.
• Provides critical care and evaluation of patients.
• Develops nursing care plans, performs care as outlined in clinical pathways, and standing order sets.
• Provides and documents individual and family education.
• Prepares sterile supplies, non-sterile supplies, and ensures required equipment is present and ready for use.
• Adjusts equipment as required. Reports all problems to the Non-Commissioned Officer In Charge
  (NCOIC) and works with maintenance service technicians to address problems.
• Checks the operating parameters of the equipment each time before the equipment is used.
• Develops and monitors quality improvement programs with peer reviews, staff education, chart reviews, and statistics.
• Performs telephone consults, triage, and other necessary administrative tasks as required,
• Maintain knowledge of how to provide nursing care from a multi-disciplinary continuum, from normal physiological changes to a wide variety of disease processes that may complicate the antepartum, intra-partum or postpartum stages of pregnancy, the immediate care of the newborn and have a varied knowledge in women’s health issues and the physiological changes in the normal life cycle.
• Maintain knowledge of the variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required.
• Maintain knowledge of the administrative requirements for proper documentation of patient’s condition including disease progress, acknowledgement of teaching, and follow up care.
• Maintain knowledge of computer operations and proficiency in the use of basic word processing, data entry and automated medical records.
• Maintain knowledge of effective communication and collaboration with the health care team, both in oral and written form.
• Maintain knowledge of synthesizing data from a variety of sources to make appropriate clinical decisions.
• The general scope of work under this contract includes performing services in the following patient care areas: Antepartum Labor & Delivery Postpartum Neonatal via Rooming-In or Nursery Obstetric Triage
• The provider shall prepare all medical documentation to meet or exceed the standards required under AFI 46-101 Nursing Services and Operations, Association of Women’s Health, Obstetric and Neonatal Nurses (AWHONN), applicable Air Force Instructions and the Joint Commission guidelines. Only approved abbreviations shall be used for documentation of care in the health care record. The Department of Defense’s electronic medical record systems, MHS Genesis shall be used for all patient encounters.
• Provide nursing care to routine and high-risk Obstetrical patients including newborns during the transition period within assigned inpatient unit based on relevant specialty care standards.
• Establish priorities of patient care based on triage, nursing diagnosis, essential patient needs and outcomes.
• Collaborate with multi-disciplinary team in assessing the effectiveness of existing therapies and interventions for patients with illnesses, their families and significant others.
• Apply understanding and operation of specialty equipment. Maintain a safe patient care environment. Monitor and troubleshoot equipment assuring appropriate patient response. Provide intervention when indicated.
• Ensure upkeep, perform checks and maintain appropriate logs of medical equipment.
• Administer medications and IV therapy employing knowledge of pharmacological agents and blood/blood components, their indications, routes of administration, expected actions and potential side effects, as related to specific patient population served. Manage complex central and peripheral intravenous lines/infusions.
• Provide recovery care for post-anesthesia patients following C-Sections, cerclage, tubal ligations, and spontaneous pregnancy loss.
• Serve as a patient advocate and maintain patient’s right to confidentiality. Assist patients, families, and significant others in identifying limitations.
• Provide telephone advice based on clinical knowledge and accepted standards. Assist as needed in accessing appropriate ambulatory/emergency care for beneficiaries.
• Consult and work collaboratively with social work and other ancillary services at the medical center and in the community to aid patients in maintaining independence, dignity, and quality of life.
• Evaluate effectiveness of own performance.
• Contribute to health care system and support clinical environment resulting in quality services and positive patient outcomes.
• Assist management and staff to develop and implement standards of care in keeping with unit and command’s strategic patient care goals.
• Demonstrate clinical expertise and independent practice necessary to function as a resource for medical and nursing personnel and mentor to staff. Provide clinical consultation and participate in problem-solving efforts of nursing staff, medical staff and other disciplines.
• Conduct and participate in patient bedside rounds with care team, patient care conferences and discharge planning.
• Assess and address the learning needs of patients and families, as well as, cultural and spiritual needs, individualizing each plan of care to meet patients’ specific needs.
• Participate in formal patient education and development of patient education materials.
• Monitor patient responses to medications/procedures/treatments and provide timely, accurate, and concise documentation, manually and/or by use of computer.
• Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
• Assist in the development, application and monitoring of clinical pathways as directed by the government.
• Assist with the development of programs that comply with all regulatory agencies and Joint Commission requirements with a focus on CI, professional practice, and patient care/teaching.

Qualifications

Necessary Skills and Knowledge:

• Maintain a level of productivity comparable with that of other individuals performing similar services.
  
• Comply with the standards of The Joint Commission, applicable provisions of law, and the rules and regulations of any and all governmental authorities pertaining to licensure and regulation of health care personnel and medical treatment facilities, the regulations and standards of medical practice and the MTF medical staff bylaws.
• Adhere to and comply with the Department of the Navy, Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract.
• Participate in peer review and performance improvement activities.
• Practice aseptic techniques as necessary; comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.
  
• Function with an awareness and application of safety procedures.
  
• Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.
  
• Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
  
• Maintain an awareness of responsibility and accountability for own professional practice.
  
• Maintain DoD email account as directed by government supervisor.
  
• Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.
  
• The HCW shall comply with the HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility. Providers shall obtain/maintain a National Provider Identifier (NPI) in accordance with DOD and MTF policy/instruction, as applicable.
• Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required.
  
• Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies.
  
• Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook.

Minimum Qualifications:

• Possess a Baccalaureate Degree in Nursing or an Associate Degree in Nursing or a Diploma from a Nursing Program accredited by the National League for Nursing Accrediting Commission (NLNAC).
• Minimum of two (2) year experience as an inpatient labor and delivery nurse is required. Experience with postpartum and newborn nursery is highly recommended. All experiences should be within the last
  4 years. Have a work experience for Level 1 Labor and Delivery, American College of Obstetricians and Gynecologists (ACOG) for reference level of maternal care is highly recommended.
• Advanced Cardiac Life Support (ACLS), Neonatal Resuscitation Program (NRP), Intermediate Fetal Heart Monitoring including, but not limited from Association of Women’s Health Obstetrics and Neonatal Nurse (AWHONN), GE healthcare, National Certification Corporation (NCC) test for Electronic Fetal Monitoring (EFM), and the Perinatal Quality Foundation. Basic Life Support (BLS) certificates should be reviewed by MDG personnel.
• Obtain and Defense Health Agency (DHA) competencies for labor and delivery nurses within 30-60 days, as well as certifications and competencies such as NMRTC Yokosuka nurse corps competencies, unit/departmental competencies, command orientation (e.g., IV, medications and blood administration) in accordance with the directorate nursing services and base-wide command policy not later than 60-90 days after commencement of services, including MTF observation of three completed IV sticks during this time.
• Emergency Clinical Obstetrics (ECO), Sugar, Temperature, Airway, Blood pressure, Lab work, and Emotional support (STABLE) is mandatory within 30-90 duty days of arrival.
• Possess and maintain a current unrestricted license to practice as a registered nurse in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands
  
• HCW shall be in good standing, and under no restrictions, with the state licensure board in any state in which a license is held or has been held within the last 10 years.
  
• HCW shall be in good standing and under no sanction or suspension by the Federal Government.
  
• Possess a Neonatal Resuscitation Provider (NRP) certifications. HCWs not currently in possession of current certification must acquire certification prior to initiating contract performance. Web based classes do not meet these standards.
• Able to read, write, speak, and understand the English language fluently and maintain good communication skills with patients and other health care personnel.
  
• Ability to pass all background/suitability checks.
  
• A valid driver’s license; ability to travel locally as needed.
• Must be a US Citizen 

Preferred Qualifications:

• A thorough understanding of the military lifestyle and experience working with military family members.
  
• Working knowledge of relevant military, state, federal, and local laws and resources.
  
• Highly developed written, oral, and presentation communication skills
• An active Security Clearance is preferred.
