The Associate Care Navigator provides service coordination, accompaniment, and transportation to medical appointments for adults. They collaborate with the Care Navigation team to meet client lifeplan goals and advocate for clients during discharge and care planning meetings.
A bachelor's degree is preferred, though candidates with healthcare experience will be considered. Applicants must possess a valid Rhode Island driver's license and be willing to drive clients.
